歴史とあらまし

 ヒトや動物の脳下垂体には、GH分泌を促す受容体(GHSR)の存在が報告されており(Ref 1)、1999年、ラットの胃から発見された28個のアミノ酸からなるグレリンがその内因性リガンドであることがわかった(Ref 2)。GHSRはG protein-coupled receptorで遺伝子は3番染色体上に位置する。

分子構造

 7回膜貫通型G-protein coupled receptor 1 familyに属し、366個のアミノ酸からなる。

機能

 GHSRの内因性リガンドであるグレリンは、食欲抑制効果をもつレプチンやCCKなどと逆の食欲を増進させる効果を持つ。血中のレプチン濃度はBMIと相関するがグレリン濃度は逆相関することが報告されている(Ref 3)。

老化・老年病における意義

  高齢者では、些細な原因で、食欲不振、栄養障害が生じやすくなっており(Ref 4)エネルギーバランスの維持のためにも、適切な摂食調節が重要である。GHSRには7種類の遺伝子多型が報告されている(Ref 5)。そのうち、171TT→CC変異が摂食障害のひとつbulimia nervosaと有意の関わりがみられた(Ref 6)。
老齢ラットやマウスでは、グレリン投与による食欲増進効果が低下または消失しており、おそらく、受容体(GHSR)の発現低下あるいは機能低下が原因と考えられる(Ref 7-9)。
