研究紹介

1.認知症の早期診断法および発症予測法の開発

健常老年者脳画像を追跡し、多数例の画像データベースを構築しています。PET薬剤科学テーマと協力して、脳の形態と機能の変化を感度良く検出するツールを開発し、正常な加齢変化の姿を明らかにするとともに、認知症の早期あるいは発症前の変化を検出することを目指しています。また、高齢者ブレインバンクと連携して、病理診断に基づいたPET画像の意義づけの検討も行っています。こうした双方向的な画像研究により、現在精力的に取り組んでいるアルツハイマー病診断の他、レビー小体型認知症、前頭側頭型認知症等の非アルツハイマー型認知症の診断法の開発にも取り組んで行きます。

2.アミロイド・タウイメージングの実用化に向けた研究

私たちは11C-PiBを2005年に導入して以来、アミロイドPET診断技術の実証研究、多施設研究、治験を行い、検査の標準化や読影・解析法の開発、適正使用ガイドラインの策定にも携わり、わが国におけるアミロイドPETの診療実装に大きく貢献してきました。また、18F-MK6240によるタウPET診断技術を立ち上げ、臨床研究を推進しています。アルツハイマー病の中核病理である老人斑と神経原線維変化の脳内進展を可視化できるこれらの検査法は、アルツハイマー病の病態理解、治療薬開発、治療薬の効果判定に欠かせない重要な診断技術です。これからも認知症の根本治療法、予防法開発に向けて研究を推進してゆきます。

アミロイド病理は認知機能性上のうちに立ち上がり、出現してから認知症発症までは20年以上の時間がかかると言われている。アミロイド病理が広がるとタウ病理が出現し、神経細胞障害を引き起こし、認知機能障害の発症に至る。アミロイドとタウの評価により、客観的で正確な病期診断、発症進行予測が可能になる。

3.PET診断技術の老年学臨床研究および診療への応用

PET診断技術を用いて、ヒトの脳機能を循環、代謝、神経受容体等に基づいて解析することにより、老化や老人病の病態を明らかにし、臨床的に有用なPET診断法を提案、普及することを目指しています。特に認知症・パーキンソン症候群などの疾患の早期診断法や鑑別診断法の普及に重点を置いていますが、脳虚血、てんかん、不随意運動疾患、脳腫瘍などの診断研究も展開しています。図3は現在精力的に研究しているアストログリオーシス画像による病態診断を示します。

18F-THK5351は東北大学によりタウPET診断薬として開発されたが、MAO-B(モノアミン酸化酵素B型)に強い結合性を有し、グリア細胞(アストロサイト)が活性化したときに高発現されるMAO-Bを感度良く可視化できる。これにより、脳損傷に伴うグリオーシスを陽性信号として明瞭に検出でき、様々な変性疾患、血管障害、炎症性疾患、腫瘍の診断に有用であることを報告してきた。18F-SMBT-1は東北大学によって開発されたMAO-Bに特異性の高い診断薬で、次世代のアストログリオーシス診断薬として期待される。
